1) PAY
Pay training fee/s and record your order ID. You will need this when filling in the survey (e.g. 133).
Select how many members you are paying for. There is an automatically calculated discount applied for 2 or more family members.
NOTE If you have previously paid for family members choose either 2nd or 3rd member payments.
2) FILL IN THIS FAMILY MEMBER SURVEY
1 survey per family member. Please do not repeatedly fill in the survey for the same member.
Once you have clicked Submit then it has gone through. If in doubt, see below.
